Online Registration
To begin the registration process, please call us at (925) 277-2050 to schedule an appointment. We will email you a link to our online registration form through our secure web server. At your convenience, you will be able to fill out all the patient forms at home before your appointment. At the time of your appointment, we will download your form directly into our electronic medical record. If you don't have an email address, please arrive 15 minutes early to your appointment to fill out your registration form.
At Your First Visit
Please remember to bring your record of past immunizations and your yellow vaccination record if you have one. It is also helpful to bring any pertinent medical history, and your specific itinerary. This information is important to help us make recommendations regarding which immunizations your will need for your trip. Please allow 30-60 minutes for your first visit which includes a travel consultation and vaccine administration at our clinic.
Cost/Insurance Information
The price for a travel consult is $50. The cost of the consult will be split between travelers that come to the clinic at the same visit. The vaccines are priced separately. If you need only a booster for travel, school or work, you will not be charged a consult, just the price of the vaccine. For returning travelers that have not been seen in our clinic for 2 years or more, there will be a $25 office visit fee.
We are not contracted with any insurance plans and ask for payment at the time of your visit. We accept all credit cards, checks, and cash. Many insurance plans do not cover travel vaccines, however, you will be provided with a statement which you can submit to your insurance for possible reimbursement.
