At Your First Visit
If you have not registered with us before your appointment, please arrive a few minutes early to complete the registration form. Please remember to bring your record of past immunizations and your yellow vaccination record if you have one. It is also helpful to bring any pertinent medical history, and your specific itinerary. This information is important to help us make recommendations regarding which immunizations you will need for your trip.

Cost/Insurance Information
The price for a travel consultation is $50. The cost of the consultation may be split between travelers that come to the clinic together. The vaccines are priced separately. If only a booster is needed for travel, school or work, you will not be charged a consultation, just the price of the vaccine.
Health plans vary in what they cover. We are not contracted with any insurance plans and ask for payment at the time of your visit. We accept credit cards, checks, and cash. A statement will be provided that can be submitted to your insurance provider for possible reimbursement.
